2. Click on "Create an MTA".
3. From the drop-down Affiliation Menu, select "JHU".
4. From the drop-down Department Menu, select "SOM Medicine"
5. Click "Next".
6. From the drop-down Provider Menu, select "William Bishai".
7. Click "Next", and follow the displayed instructions.
8. Please print and sign the generated MTA document.
9. Fax the completed form to Julia Brill at 410-516-6499.
